Xtrapel 100 mg Injection is a potent, fast-acting analgesic designed for the rapid management of moderate to severe acute pain. It is administered via injection for quicker onset of action, making it suitable for post-operative pain, acute injury pain, and other conditions requiring immediate pain relief.
Uses & Indications
Dosage
Adults
100 mg administered intravenously (IV) or intramuscularly (IM) every 6-8 hours as needed. Maximum dose 400 mg/day.
Elderly
A reduced dose of 50 mg every 8-12 hours may be considered, with careful monitoring for adverse effects.
Renal_impairment
For moderate impairment (CrCl 30-60 mL/min), reduce dose to 50 mg every 8-12 hours. Not recommended for severe impairment (CrCl < 30 mL/min).
How to Take
Xtrapel 100 mg Injection should be administered slowly over 2-3 minutes if given intravenously, or deep intramuscularly if given IM. Do not mix with other medications in the same syringe.
Mechanism of Action
Xtrapel selectively modulates neuronal pain pathways in the central nervous system, reducing the transmission of pain signals without acting on opioid receptors. It also exhibits mild anti-inflammatory properties by inhibiting certain inflammatory mediators at the site of injury.

Excretion
Mainly excreted via urine (70%) and feces (30%) within 24 hours.
Half life
Approximately 3-4 hours.
Absorption
Rapidly and completely absorbed after intramuscular or intravenous administration, reaching peak plasma concentrations within 15-30 minutes.
Metabolism
Primarily metabolized in the liver via cytochrome P450 enzymes (e.g., CYP3A4, CYP2D6) into inactive metabolites.
Side Effects
Contraindications

Drug Interactions
Alcohol
May increase CNS depressant effects and risk of liver toxicity.
Warfarin
May enhance anticoagulant effect, increasing bleeding risk. Monitor INR closely.
Other Analgesics
Concurrent use with other analgesics may increase the risk of additive side effects without increasing efficacy. Avoid combination.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe dizziness, drowsiness, confusion, nausea, vomiting, and potentially seizures. Treatment is symptomatic and supportive, including gastric lavage (if oral intake), activated charcoal, and maintaining vital functions. There is no specific antidote.
Pregnancy & Lactation
Use in pregnancy should be avoided unless clearly necessary, as safety has not been established. It is not known if Xtrapel is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother.
