Overview of the medicine
Xtrapel 50 mg capsule contains Pregabalin, an antiepileptic drug used to treat various types of neuropathic pain, epilepsy (as an add-on therapy), and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose is typically 75 mg twice daily or 50 mg three times daily (150 mg/day). Dose can be increased based on response, up to a maximum of 300 mg twice daily or 200 mg three times daily (600 mg/day). Doses should be titrated gradually.
Elderly
No specific dose adjustment is required solely based on age. However, dose adjustments may be necessary due to age-related decline in renal function. Monitor renal function.
Renal_impairment
Dosage must be individualized based on creatinine clearance. Significantly reduced doses are required for severe renal impairment.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not chew, crush, or break it.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta (α2-δ) subunit of voltage-gated calcium channels in the central nervous system. This binding modulates the release of several excitatory neurotransmitters, thereby reducing neuronal excitability and pain transmission.
Pharmacokinetics
Onset
Pain relief may be observed within one week, with full therapeutic effects in 2-4 weeks. Anxiolytic effects may be seen earlier.
Excretion
Primarily excreted unchanged in urine. Renal excretion is the major elimination route.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1 hour. Bioavailability is ≥ 90% and is independent of dose.
Metabolism
Undergoes negligible metabolism in humans. Less than 0.1% of the dose is recovered as metabolites in urine.
Side Effects
Contraindications
- •Hypersensitivity to pregabalin or any of its excipients.
Drug Interactions
Angiotensin-converting enzyme (ACE) inhibitors
Potential for enhanced peripheral edema when co-administered with pregabalin.
Opioids, Benzodiazepines, Alcohol, Barbiturates
May enhance CNS depressant effects, increasing the risk of somnolence, respiratory depression, and sedation.
Storage
Store below 30°C in a dry place, away from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and rarely, coma. Treatment is supportive and symptomatic; hemodialysis may be beneficial in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted in human milk; caution should be exercised when administered to a nursing woman.
