Overview of the medicine
Zemicef Max (Cefixime) is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, and otitis media. It works by inhibiting bacterial cell wall synthesis.
Uses & Indications
Dosage
Adults
Adults: 400 mg once daily or 200 mg twice daily. The duration of treatment depends on the infection.
Elderly
No specific dose adjustment is required for elderly patients unless they have severe renal impairment.
Renal_impairment
For patients with creatinine clearance < 60 mL/min, dosage adjustment is necessary. Consult product leaflet for specific guidelines.
How to Take
Zemicef Max can be taken with or without food. For best absorption and to minimize gastrointestinal upset, it may be taken with food.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to the lysis and death of bacterial cells.
Pharmacokinetics
Onset
Onset of action is relatively rapid.
Excretion
Primarily renal, with approximately 50% of an absorbed dose excreted unchanged in urine within 24 hours. Some biliary excretion also occurs.
Half life
Plasma half-life is typically 3-4 hours, potentially longer in elderly or renally impaired patients.
Absorption
Approximately 40-50% orally absorbed. Absorption is not significantly altered by food.
Metabolism
Limited hepatic metabolism; a significant portion is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to penicillin.
Drug Interactions
Warfarin
May increase prothrombin time; monitor INR closely.
Probenecid
Increases cefixime plasma concentrations by inhibiting renal excretion.
Carbamazepine
May increase carbamazepine levels; monitor for toxicity.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

In case of overdose, gastric lavage may be indicated. Treatment should be symptomatic and supportive. Hemodialysis or peritoneal dialysis does not remove Cefixime in significant quantities.
