Description
Overview of the medicine
Metronidazole is an antibiotic and antiprotozoal medication used to treat a wide variety of infections, including parasitic infections, bacterial vaginosis, and infections caused by anaerobic bacteria.
Uses & Indications
Dosage
Adults
Amebiasis: 500-750 mg orally 3 times daily for 5-10 days. Trichomoniasis: 2 g as a single dose or 500 mg twice daily for 7 days. Bacterial Vaginosis: 500 mg orally twice daily for 7 days. Anaerobic infections: 500 mg orally 3 times daily for 7-10 days.
Elderly
Dosage adjustment may be considered in elderly patients with significant hepatic impairment, otherwise standard adult dose.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment; for severe (creatinine clearance <10 mL/min), consider reducing dosage by 50% or extending dosing interval.
How to Take
Orally, preferably with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water.
Mechanism of Action
It inhibits nucleic acid synthesis by disrupting the DNA of microbial cells, leading to cell death. It is effective against anaerobic bacteria and certain protozoa.
Pharmacokinetics
Onset
Within hours, depending on infection and dosage form.
Excretion
Mainly renal (60-80% as metabolites), some fecal (6-15%).
Half life
Approximately 8 hours (range 6-12 hours).
Absorption
Rapid and almost complete oral absorption. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver by hydroxylation, oxidation, and glucuronidation.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives
- •Concurrent use of disulfiram (within 14 days)
- •Concurrent use of alcohol or propylene glycol
Drug Interactions
Alcohol
Disulfiram-like reaction (flushing, palpitations, nausea, vomiting, headache, hypotension). Avoid during and for at least 3 days after treatment.
Lithium
May increase serum lithium levels, leading to toxicity.
Disulfiram
Psychotic reactions; avoid concurrent use within 14 days.
Phenytoin, Phenobarbital
May decrease metronidazole plasma levels.
Warfarin and other oral anticoagulants
May enhance anticoagulant effect, monitor INR/PT closely.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of the reach and sight of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, seizures, peripheral neuropathy. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Generally not recommended in the first trimester. Use in second and third trimesters only if clearly needed. Excreted into breast milk; advise against breastfeeding during treatment and for 12-24 hours after the last dose.
