Pediatric
For infants and young children, 10-15 mg/kg body weight, every 4-6 hours as needed. Do not exceed 5 doses in 24 hours. Refer to specific age/weight dosage chart: 0-3 months (2.7-5.3 kg): 0.6 ml (48 mg) to 0.8 ml (64 mg); 4-11 months (5.4-8.0 kg): 1.0 ml (80 mg) to 1.4 ml (112 mg); 1-2 years (8.1-12 kg): 1.6 ml (128 mg) to 2.4 ml (192 mg).
Renal_impairment
Not applicable for this pediatric formulation. Dose adjustment may be required for other paracetamol forms in severe renal impairment.
How to Take
Administer orally using the provided dropper for accurate measurement. Can be given with or without food. Shake the bottle well before use.
Mechanism of Action
Paracetamol works by inhibiting prostaglandin synthesis, primarily in the central nervous system, which helps to reduce fever and pain. It acts on the hypothalamic heat-regulating center to produce antipyresis.
Pharmacokinetics
Onset
Onset of analgesic action within 30-60 minutes; antipyretic action within 30 minutes.
Excretion
Mainly excreted by the kidneys as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma half-life is 1 to 4 hours, may be prolonged in liver disease or overdose.
Absorption
Rapid and complete absorption from the gastrointestinal tract.
Metabolism
Primarily metabolized in the liver mainly by conjugation with glucuronic acid and sulfate. A small amount is metabolized by cytochrome P450 to a toxic intermediate, NAPQI.
Side Effects
Contraindications
- •Hypersensitivity to paracetamol or any component of the formulation.
- •Severe hepatic impairment or active liver disease.
Drug Interactions
Cholestyramine
Reduces the absorption of paracetamol if given within one hour.
Warfarin and other coumarins
May enhance the anticoagulant effect, leading to an increased risk of bleeding, especially with prolonged and high-dose paracetamol use.
Metoclopramide and domperidone
May increase the rate of absorption of paracetamol.
Carbamazepine, phenobarbital, phenytoin, rifampicin
May increase the risk of paracetamol hepatotoxicity.
Storage
Store below 30°C in a dry place, away from direct sunlight. Keep out of reach of children. Do not freeze.
Overdose
Overdose can cause severe, sometimes fatal, liver damage. Symptoms include nausea, vomiting, abdominal pain, and sweating. Management involves gastric lavage, activated charcoal, and administration of the antidote N-acetylcysteine (NAC) as soon as possible.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation at recommended doses. However, since this is a pediatric formulation, this advice is for general context and not direct usage by pregnant/lactating individuals.
