Overview of the medicine
Zico 20 mg tablet contains Esomeprazole, a proton pump inhibitor that reduces stomach acid production. It is used to treat conditions like heartburn, acid reflux, and ulcers.
Uses & Indications
Dosage
Adults
For GERD: 20 mg or 40 mg once daily for 4-8 weeks. For Erosive Esophagitis: 20 mg or 40 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily for 7-10 days in combination with antibiotics.
Elderly
No dosage adjustment generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment required.
How to Take
Take the tablet whole with a glass of water, at least one hour before a meal. Do not crush, chew, or split the tablet.
Mechanism of Action
Esomeprazole is an S-isomer of omeprazole and acts as a proton pump inhibitor. It specifically inhibits the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells, thereby blocking the final step in acid production.

Mainly excreted in urine (approximately 80%) and feces (approximately 20%) as metabolites.

Approximately 1.3 hours
Absorption
Rapidly absorbed after oral administration; bioavailability is about 64% with a single 40 mg dose and increases to 89% with repeated once-daily administration. Food delays and decreases absorption.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole or any components of the formulation
- •Concomitant use with nelfinavir
Drug Interactions
Digoxin
Increased absorption and plasma concentration of digoxin.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma levels of methotrexate.
Nelfinavir, Atazanavir
Decreased plasma levels of these antivirals, concomitant use is contraindicated with nelfinavir.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include gastrointestinal disturbances and fatigue. Management is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, so caution should be exercised when administered to a nursing mother.
