Overview of the medicine
Zidimax 250 mg Injection contains Azithromycin, a macrolide antibiotic used to treat a wide range of bacterial infections. It is administered intravenously for severe infections or when oral therapy is not suitable.
Uses & Indications
Dosage
Adults
500 mg IV as a single daily dose for 1-2 days, followed by oral azithromycin therapy. For PID, 500 mg IV as a single daily dose for 1-2 days, followed by 250 mg oral azithromycin daily to complete a 7-day course.
Elderly
No specific dosage adjustment is recommended, but caution should be exercised in elderly patients with pre-existing cardiac conditions due to potential QTc prolongation.
Renal_impairment
No dosage adjustment is needed for mild to moderate renal impairment (creatinine clearance >10 mL/min). Use with caution in severe renal impairment as data is limited.
How to Take
Administer as an intravenous (IV) infusion over a period of 60 minutes. Do not administer as a rapid intravenous bolus or intramuscular injection. The powder should be reconstituted with sterile water for injection and then diluted in an appropriate intravenous solution before administration.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby blocking the translocation of peptides. This action is primarily bacteriostatic, but can be bactericidal at high concentrations.

Onset
Peak plasma concentrations are achieved shortly after the end of the 1-hour infusion.
Excretion
Mainly excreted unchanged in bile. A small amount (approximately 6%) is excreted via renal route.
Half life
Terminal elimination half-life is approximately 2-4 days due to extensive tissue distribution.
Absorption
Intravenous administration ensures 100% bioavailability, bypassing gastrointestinal absorption.
Metabolism
Primarily metabolized in the liver to inactive metabolites; however, a significant portion is excreted unchanged.
Side Effects
Contraindications
- •Known hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Pimozide
Concomitant use is contraindicated due to risk of QTc prolongation and arrhythmias.
Warfarin
Potentiation of anticoagulant effect, monitor PT/INR.
Nelfinavir
Increased serum concentrations of azithromycin, monitor for adverse effects.
Cyclosporine
Increased cyclosporine levels, monitor cyclosporine concentrations and adjust dose.
Antacids (containing aluminum or magnesium)
May reduce peak plasma concentrations (primarily for oral formulations, but should be considered).
Storage
Store powder vials below 30°C, protected from light and moisture. Do not freeze. Refer to specific manufacturer instructions for reconstituted and diluted solution storage.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves supportive measures, gastric lavage, and symptomatic treatment as required.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Azithromycin is excreted in breast milk; caution should be exercised when administered to a nursing mother.
