Azithromycin is a macrolide antibiotic used to treat various bacterial infections. This 500 mg injection is administered intravenously for severe infections.
Uses & Indications
Dosage
Adults
For CAP: 500 mg IV as a single daily dose for 1 to 2 days, followed by oral azithromycin once daily to complete a 7- to 10-day course. For PID: 500 mg IV as a single daily dose for 1 to 2 days, followed by oral azithromycin once daily to complete a 7-day course.
Elderly
No specific dosage adjustment is recommended for elderly patients; however, monitor for adverse effects due to potential age-related organ function decline.
Renal_impairment
No dosage adjustment is required for mild to moderate renal impairment. Use with caution in severe renal impairment (creatinine clearance <10 mL/min) due to lack of data.
How to Take
Zidimax 500 mg injection should be administered as a slow intravenous infusion over a period of 60 minutes or longer. It must be reconstituted and diluted according to the manufacturer's instructions before use. Do not administer as a bolus or intramuscularly.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby interfering with the translation of mRNA.

Excretion
Primarily excreted unchanged in the bile; a small amount is excreted via the kidneys.

Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.

Drug Interactions
Digoxin
Increased serum digoxin levels, monitor digoxin levels.
Warfarin
Increased anticoagulant effect, monitor INR.
Nelfinavir
Increased azithromycin concentrations.
Cyclosporine
Increased serum cyclosporine levels, monitor cyclosporine levels.
QT prolonging drugs
Additive risk of QT prolongation (e.g., antiarrhythmics, antipsychotics).
Storage
Store the unopened vial at controlled room temperature, below 30°C (86°F), protected from light and moisture. Reconstituted and diluted solutions should be used promptly and not refrigerated.
Overdose
Symptoms of azithromycin overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and supportive care as required.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. Excreted in human milk; caution should be exercised when administered to a nursing woman.
