Description
Overview of the medicine
Omeprazole is a proton pump inhibitor used to reduce stomach acid production. It is used to treat conditions like heartburn, acid reflux, and stomach ulcers.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For Duodenal/Gastric Ulcers: 20 mg once daily for 2-4 weeks. For Zollinger-Ellison Syndrome: Initial 60 mg once daily, adjusted as needed.
Elderly
No specific dose adjustment required, but caution is advised in elderly patients with hepatic impairment.
Renal_impairment
No dose adjustment needed for renal impairment.
How to Take
Take orally, preferably in the morning, at least 30 minutes before a meal. Tablets should be swallowed whole; do not chew or crush. For patients with swallowing difficulties, MUPS tablets can be dispersed in water or fruit juice.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby suppressing gastric acid secretion.

Onset
Within 1 hour
Excretion
Mainly excreted in urine (approximately 80%) and feces (approximately 20%) as metabolites.
Half life
0.5 to 1 hour (plasma), but pharmacological effect lasts longer due to irreversible binding.
Absorption
Rapidly absorbed from the gastrointestinal tract, though absorption may be delayed by food. Bioavailability is about 30-40% after a single dose, increasing to about 60% with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to omeprazole or any component of the formulation
- •Concomitant use with nelfinavir
Drug Interactions
Digoxin
Increased digoxin absorption; monitor digoxin levels.
Warfarin
Increased INR/prothrombin time; monitor closely.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel (via CYP2C19 inhibition).
Methotrexate
Increased methotrexate levels; consider temporary PPI discontinuation.
Diazepam, Phenytoin
Prolonged elimination of these drugs due to CYP2C19 inhibition.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms: Nausea, vomiting, abdominal pain, diarrhea, headache, lethargy. Treatment: Symptomatic and supportive care. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Excreted in human milk, so caution advised during breastfeeding.
