Overview of the medicine
Zo-mups 40 mg Tablet is a proton pump inhibitor (PPI) used to treat conditions where the stomach produces too much acid, such as gastroesophageal reflux disease (GERD), erosive esophagitis, and Zollinger-Ellison syndrome. It works by reducing the amount of acid your stomach makes.
Uses & Indications
Dosage
Adults
Typically 40 mg once daily, preferably in the morning before breakfast, for 4-8 weeks. For Zollinger-Ellison, higher doses may be required.
Elderly
No specific dose adjustment is usually required for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment or those on hemodialysis.
How to Take
Take the tablet whole, do not crush or chew. It should be taken orally, preferably 30-60 minutes before a meal, usually in the morning.
Mechanism of Action
Pantoprazole is a proton pump inhibitor that irreversibly binds to the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, thereby inhibiting the final step in gastric acid secretion. This leads to a dose-dependent inhibition of both basal and stimulated gastric acid secretion.

Onset
Antisecretory effect within 2.5 hours.
Excretion
Mainly excreted renally (approx. 71%), with a smaller portion excreted in feces (approx. 18%).
Half life
Approximately 1 hour (terminal elimination half-life is longer, but not directly related to duration of action).
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached within 2-2.5 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole or any component of the formulation.
- •Hypersensitivity to other substituted benzimidazoles.
Drug Interactions
Warfarin
Increased INR and prothrombin time. Monitor closely.
Clopidogrel
Potential for reduced antiplatelet effect of clopidogrel (though clinical significance is debated).
Methotrexate
May increase methotrexate levels, especially with high-dose therapy.
Drugs dependent on gastric pH for absorption (e.g., Ketoconazole, Atazanavir)
Reduced absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include mild gastrointestinal disturbances. Treatment is symptomatic and supportive. Pantoprazole is not significantly removed by hemodialysis.
