Description
Overview of the medicine
Omeprazole is a proton pump inhibitor (PPI) that reduces gastric acid secretion. It is used to treat conditions such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Usually 20 mg once daily before breakfast for 4-8 weeks for GERD and ulcers. For Zollinger-Ellison syndrome, initial dose is 60 mg once daily, adjusted as needed.
Elderly
No specific dosage adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take Zolat 20 mg capsule orally, preferably in the morning before a meal. Swallow the capsule whole with water; do not chew, crush, or open it.
Mechanism of Action
Omeprazole irreversibly binds to and inhibits the H+/K+-ATPase (proton pump) in the secretory canaliculi of gastric parietal cells. This action blocks the final step in gastric acid production, leading to a profound and long-lasting reduction in gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression occurs within 1-2 hours. Full therapeutic effect within 4 days of repeated dosing.
Excretion
Approximately 77% of the dose is excreted in the urine as metabolites, and about 18% is excreted in the feces.
Half life
The plasma elimination half-life is short, about 0.5 to 1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached within 0.5 to 3.5 hours. Bioavailability is approximately 30-40%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased and prolonged elevation of methotrexate and/or its metabolite serum levels.
Nelfinavir, Atazanavir
Reduced plasma concentrations of nelfinavir and atazanavir, leading to loss of therapeutic effect.
Warfarin, Diazepam, Phenytoin
Increased plasma concentrations of these drugs, requiring monitoring and potential dose adjustment.
