Description
Overview of the medicine
Zolat 40 mg Capsule contains Omeprazole, a proton pump inhibitor, used to reduce stomach acid and treat conditions like acid reflux, ulcers, and heartburn.
Uses & Indications
Dosage
Adults
Usually 20-40 mg once daily, depending on the condition. For severe GERD or Zollinger-Ellison Syndrome, higher doses may be required.
Elderly
No specific dosage adjustment is typically required based on age alone, but monitor for adverse effects.
Renal_impairment
No dosage adjustment required in patients with renal impairment.
Hepatic_impairment
Dose adjustment may be necessary in patients with severe hepatic impairment; a daily dose of 20 mg may be sufficient.
How to Take
Take Zolat 40 mg Capsule orally, preferably in the morning before a meal. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme system (the proton pump) in the secretory canaliculi of the gastric parietal cells. This reduces gastric acid secretion by blocking the final step of acid production.
Pharmacokinetics
Onset
Onset of acid suppression within 0.5-3.5 hours.
Excretion
Approximately 77% of the dose is excreted in the urine as metabolites, and the remainder in the feces.
Half life
Plasma elimination half-life is short, about 0.5-1 hour, but the duration of action is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed from the gastrointestinal tract. Bioavailability is approximately 30-40% after a single dose, increasing slightly with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzymes CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
May increase digoxin levels due to increased gastric pH.
Warfarin
May increase INR and prothrombin time, requiring careful monitoring and dose adjustment.
Tacrolimus
May increase serum levels of tacrolimus.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting its metabolism to the active metabolite via CYP2C19.
Methotrexate
May increase serum levels of methotrexate, especially at high doses, leading to potential toxicity.
St. John's Wort
May decrease omeprazole levels.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Omeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- For long-term use (over 1 year), consider monitoring Vitamin B12 levels.
- Monitor magnesium levels periodically in patients on prolonged PPI therapy, especially with concomitant diuretics.
Doctor Notes
- Assess for symptoms of GI malignancy before initiating PPI therapy, especially in older patients with new or worsening symptoms.
- Consider the lowest effective dose for the shortest duration necessary.
- Advise patients on potential long-term risks with prolonged use.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not crush, chew, or open the capsule.
- Inform your doctor if your symptoms do not improve or worsen.
