Description
Overview of the medicine
Sertraline is an antidepressant belonging to the Selective Serotonin Reuptake Inhibitors (SSRIs) class. It is used to treat major depressive disorder, obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).
Uses & Indications
Dosage
Adults
Typically initiated at 50 mg once daily. Dosage may be increased gradually, if necessary, to a maximum of 200 mg/day.
Elderly
Start with a lower dose (e.g., 25 mg once daily) and adjust cautiously, monitoring for side effects.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally once daily, with or without food. It is recommended to take it at the same time each day.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons, thereby potentiating serotonergic activity in the central nervous system.

Onset
Therapeutic effects typically begin within 2 to 4 weeks of regular treatment, though some improvement may be seen earlier.
Excretion
Approximately 40-45% excreted in urine and 40-45% in feces, mainly as inactive metabolites.
Half life
Approximately 26 hours (range 22-36 hours) for sertraline and 62-104 hours for its active metabolite, N-desmethylsertraline.
Absorption
Well absorbed orally, with peak plasma concentrations occurring 4.5 to 8.4 hours after administration. Food slightly increases absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, CYP2B6, CYP2C9, CYP2D6, and CYP3A4 enzymes, to its major active metabolite, N-desmethylsertraline.
Side Effects
Contraindications
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of stopping an MAOI.
- Concomitant use with pimozide.
- Hypersensitivity to sertraline or any component of the formulation.
Drug Interactions
Pimozide
Increased pimozide levels and QT prolongation.
Warfarin
Potentiation of anticoagulant effect, leading to increased bleeding risk.
Triptans (e.g., sumatriptan)
Increased risk of serotonin syndrome.
MAOIs (e.g., phenelzine, selegiline)
Increased risk of serotonin syndrome, which can be life-threatening.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Increased risk of bleeding, especially gastrointestinal bleeding.
Other serotonergic drugs (e.g., tricyclic antidepressants, fentanyl, lithium, tramadol, St. John's Wort)
Increased risk of serotonin syndrome.

Overdose
Symptoms of overdose include drowsiness, nausea, vomiting, tachycardia, tremor, agitation, dizziness, and rarely, seizures or coma. Management involves supportive and symptomatic treatment. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit outweighs the potential risk to the fetus. Sertraline is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother.
