Description
Overview of the medicine
Sertraline is an antidepressant belonging to the Selective Serotonin Reuptake Inhibitor (SSRI) class. It is used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, post-traumatic stress disorder, premenstrual dysphoric disorder, and social anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose is typically 50 mg once daily. May be increased in increments of 50 mg at weekly intervals, up to a maximum of 200 mg/day.
Elderly
Generally, similar to adult dosage, but lower initial doses and slower titration may be considered due to potential for increased sensitivity.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Caution advised in severe impairment due to lack of extensive studies.
How to Take
Zosert 50 mg Tablet can be taken once daily, with or without food. It is important to take it at the same time each day. Do not crush, chew, or break the tablet; swallow it whole.
Mechanism of Action
Sertraline works by selectively inhibiting the reuptake of serotonin (5-HT) into presynaptic neurons, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced serotonergic neurotransmission, which is thought to alleviate symptoms of depression and anxiety disorders.

Excretion
Approximately 40-45% excreted in urine and 40-45% in feces, largely as metabolites.
Half life
Approximately 26 hours for sertraline; N-desmethylsertraline (active metabolite) has a half-life of 62-104 hours.
Absorption
Well absorbed orally; absorption is slightly increased with food.
Metabolism
Extensive hepatic metabolism, primarily by CYP2C19, CYP2B6, CYP2D6, CYP3A4, and CYP2C9, to N-desmethylsertraline and other inactive metabolites.
Side Effects
Contraindications
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy due to risk of serotonin syndrome.
- Concomitant use with pimozide.
- Hypersensitivity to sertraline or any component of the formulation.
- Concomitant use of the oral solution with disulfiram due to alcohol content.
Drug Interactions
MAOIs
Risk of serious, sometimes fatal, serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes).
Pimozide
Increased pimozide levels and QT prolongation.
Warfarin
Increased prothrombin time/INR, potential for bleeding.
NSAIDs, Aspirin
Increased risk of bleeding.
Serotonergic drugs (e.g., triptans, fentanyl, lithium, tramadol, St. John's Wort)
Increased risk of serotonin syndrome.

Store at room temperature (below 30°C). Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tachycardia, nausea, dizziness, agitation, tremor, and coma. There is no specific antidote. Treatment should be symptomatic and supportive, including maintaining an adequate airway, oxygenation, and ventilation. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Sertraline is excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
