Overview of the medicine
Zultan (Pantoprazole) is a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions caused by too much stomach acid.
Uses & Indications
Dosage
Adults
Typically 20-40 mg once daily, depending on the condition. For GERD/erosive esophagitis, 40 mg once daily for 4-8 weeks.
Elderly
No specific dose adjustment is usually required for elderly patients with normal renal and hepatic function.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Tablets should be swallowed whole with water before a meal, usually in the morning. Do not crush or chew. IV injection should be administered by a healthcare professional.
Mechanism of Action
Pantoprazole selectively inhibits the H+/K+-ATPase enzyme system (the 'proton pump') in the secretory canaliculi of the gastric parietal cells. This irreversible inhibition blocks the final step of acid production.

Onset
Within 2.5 hours.
Excretion
Primarily excreted renally (approximately 71%) and the remainder via fecal excretion (approximately 18%).
Half life
Approximately 1 hour (plasma elimination half-life), but its anti-secretory effect lasts much longer.
Absorption
Rapidly absorbed after oral administration; bioavailability is approximately 77%.
Metabolism
Extensively metabolized in the liver via cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole, other substituted benzimidazoles, or any component of the formulation.
- •Patients with severe liver impairment should use with caution.
Drug Interactions
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
Increased methotrexate levels, especially with high doses.
HIV Antivirals
Avoid concomitant use with atazanavir and nelfinavir due to reduced absorption.
Drugs dependent on gastric pH
May alter absorption of drugs like ketoconazole, iron salts, and erlotinib.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Experience with pantoprazole overdose is limited. Symptoms are generally mild and include nausea, vomiting, dizziness, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Pantoprazole is excreted in human milk, so use with caution in nursing mothers.
