Overview of the medicine
Zycin 200 mg Suspension contains Azithromycin, a macrolide antibiotic, used to treat a wide range of bacterial infections including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
For certain adult infections like Chlamydia trachomatis, 1 gram as a single oral dose. For pediatric use (6 months and older): 10 mg/kg orally once daily for 3 days OR 10 mg/kg on Day 1 followed by 5 mg/kg orally once daily on Days 2-5. Max 500 mg per dose. (For most adult indications, tablet form is preferred.)
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment (creatinine clearance > 10 mL/min). Caution advised in severe renal impairment.
How to Take
Administer orally, usually once daily. Can be taken with or without food. Shake the suspension well before each use.
Mechanism of Action
Azithromycin works by inhibiting bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby interfering with the translocation of peptides and inhibiting bacterial growth.

Onset
Clinical improvement usually observed within 2-3 days of initiation of therapy, depending on the infection.
Excretion
Mainly excreted unchanged in bile, with a small percentage (approx. 6%) excreted in urine.

Side Effects
Contraindications
- Known hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase serum concentrations of digoxin. Monitor digoxin levels.
Antacids
Concurrent administration of antacids containing aluminum or magnesium with azithromycin suspension can reduce peak serum levels of azithromycin. Administer azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Increased anticoagulant effect when co-administered with azithromycin. Monitor prothrombin time/INR carefully.
Nelfinavir
Increased azithromycin serum concentrations. Monitor for azithromycin side effects.
Storage
Store dry powder at controlled room temperature (20°C to 25°C). After reconstitution, store at controlled room temperature (15°C to 30°C) and discard after 10 days. Do not freeze.

Symptoms of overdose are typically characteristic of macrolide antibiotics, including severe nausea, vomiting, diarrhea, and reversible hearing loss. Treatment should be symptomatic and supportive, as there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Azithromycin is excreted in breast milk; therefore, caution should be exercised when administered to a nursing woman.
