Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat various bacterial infections including respiratory tract infections, skin infections, ear infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Typical regimen: 500 mg once daily for 3 days OR 500 mg as a single dose on day 1, followed by 250 mg once daily for 4 days. For Chlamydia trachomatis: 1g as a single oral dose.
Elderly
No specific dose adjustment is generally required for elderly patients, but monitoring of renal/hepatic function may be prudent.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment (creatinine clearance >10 mL/min). Use with caution in severe renal impairment.
How to Take
Zycin-500 mg tablet can be taken orally, with or without food. For some specific formulations (e.g., oral suspension), it may be recommended to take it without food.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby blocking translation of mRNA. This action is primarily bacteriostatic.
Pharmacokinetics
Onset
Clinical effects usually within 2-3 days, depending on the infection.
Excretion
Primarily excreted unchanged in bile; a small amount is excreted in urine (approx. 6% of dose).
Half life
Terminal elimination half-life is 2-4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract, with approximately 37% bioavailability. Food may decrease absorption of immediate-release capsules, but tablets can generally be taken with or without food.
Metabolism
Partially metabolized in the liver to inactive metabolites, primarily via N-demethylation.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Warfarin
May enhance the anticoagulant effect; monitor PT/INR closely.
Nelfinavir
May increase azithromycin serum concentrations. Monitor for side effects.
QT prolonging drugs
Increased risk of QT prolongation and ventricular arrhythmias (e.g., cisapride, pimozide, antiarrhythmics Class IA and III).
Antacids (aluminum or magnesium containing)
May decrease peak plasma levels of azithromycin. Administer azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive treatment. Gastric lavage and general supportive measures may be initiated as needed.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus; however, there are no adequate and well-controlled studies in pregnant women. Use in pregnancy only if clearly needed. Azithromycin is excreted in breast milk; use with caution in nursing mothers, weighing the potential benefits against risks to the infant.
