Description
Overview of the medicine
Abetis 10 mg Tablet contains Aripiprazole, an atypical antipsychotic medication used to treat mental health conditions like schizophrenia, bipolar I disorder, and as an add-on treatment for major depressive disorder. It helps to restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial dose 10 mg or 15 mg once daily; target dose 10-15 mg/day. Bipolar I Disorder: Initial 15 mg once daily (monotherapy) or 10-15 mg once daily (adjunctive); target dose 15-30 mg/day. Major Depressive Disorder (adjunctive): Initial 2-5 mg once daily; target dose 5-15 mg/day.
Elderly
No specific dose adjustment is generally required based on age alone, but caution is advised due to potential for increased sensitivity and co-morbidities. Start with lower doses.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Abetis 10 mg tablet should be taken orally once daily, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Aripiprazole is thought to exert its efficacy through a combination of partial agonism at dopamine D2 and serotonin 5-HT1A receptors, and antagonism at serotonin 5-HT2A receptors. This unique mechanism helps to modulate neurotransmission, reducing symptoms of psychosis and mood instability.
Pharmacokinetics
Onset
Clinical effects may take several days to weeks to become fully apparent.
Excretion
Eliminated primarily through feces (55%) and urine (25%).
Half life
Approximately 75 hours for aripiprazole and 94 hours for its active metabolite dehydro-aripiprazole.
Absorption
Well absorbed after oral administration, with peak plasma concentrations (Tmax) reached within 3-5 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes CYP2D6 and CYP3A4, to its active metabolite dehydro-aripiprazole.
Side Effects
Contraindications
- •Hypersensitivity to aripiprazole or any component of the formulation.
- •Elderly patients with dementia-related psychosis due to increased risk of death.
Drug Interactions
Benzodiazepines (e.g., Lorazepam)
Concomitant use with aripiprazole IM injection may lead to additive sedative effects and orthostatic hypotension; monitor vital signs.
CYP3A4 Inducers (e.g., Carbamazepine, Rifampicin)
May decrease aripiprazole plasma concentrations, requiring a dose increase.
Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole)
May increase aripiprazole plasma concentrations, requiring a dose reduction.
Strong CYP2D6 Inhibitors (e.g., Quinidine, Fluoxetine, Paroxetine)
May increase aripiprazole plasma concentrations, requiring a dose reduction.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tachycardia, tremor, headache, and extrapyramidal symptoms. Management is primarily supportive; maintain airway, oxygenation, and ventilation. Monitor cardiac rhythm and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to antipsychotic drugs during the third trimester are at risk for extrapyramidal and/or withdrawal symptoms. Aripiprazole is excreted in human milk; advise women not to breastfeed during treatment.
