Description
Overview of the medicine
Abetis 40 mg Tablet contains Abetolol, a beta-blocker used to treat high blood pressure, angina pectoris, certain arrhythmias, and to prevent migraine headaches. It works by blocking beta-adrenergic receptors, leading to a reduction in heart rate and blood pressure.
Uses & Indications
Dosage
Adults
Initial dose is 40 mg once daily. May be increased to 80 mg once daily after one week if necessary, based on patient response and tolerability. Maximum dose 160 mg/day.
Elderly
Lower initial dose may be considered, starting with 20 mg once daily, with gradual titration based on response and tolerability.
Renal_impairment
Dose adjustment required. For creatinine clearance 15-35 ml/min, 20 mg once daily. For creatinine clearance <15 ml/min, 20 mg every other day. Hemodialysis patients should receive an additional dose after dialysis.
How to Take
Abetis 40 mg Tablet should be taken orally, with or without food, at the same time each day. Swallow the tablet whole with water; do not crush or chew.
Mechanism of Action
Abetolol selectively blocks beta-1 adrenergic receptors in the heart, reducing heart rate, myocardial contractility, and cardiac output. It also inhibits the release of renin from the kidneys, further contributing to blood pressure reduction.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Primarily excreted via the kidneys (renal excretion) as metabolites and unchanged drug.
Half life
3-6 hours
Absorption
Rapid and almost complete absorption from the gastrointestinal tract after oral administration. Bioavailability is about 50-60%.
Metabolism
Extensive hepatic metabolism (first-pass effect).

- •Second or third-degree atrioventricular (AV) block
- •Cardiogenic shock
- •Uncontrolled heart failure
- •Severe bronchial asthma or chronic obstructive pulmonary disease (COPD)

Drug Interactions
Digoxin
Increased risk of bradycardia.
Clonidine
If Abetolol and clonidine are co-administered, Abetolol should be discontinued several days before clonidine withdrawal to prevent rebound hypertension.
NSAIDs (e.g., Ibuprofen)
May reduce the hypotensive effects of Abetolol.
Insulin and oral hypoglycemic agents
May mask the symptoms of hypoglycemia (e.g., tachycardia) and potentiate the hypoglycemic effect.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
May increase the risk of bradycardia and AV block; caution advised.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe bradycardia, hypotension, acute cardiac failure, and bronchospasm. Treatment is symptomatic and supportive, including gastric lavage, activated charcoal, and intravenous fluids. Atropine, glucagon, or beta-agonists may be administered as needed.
Pregnancy & Lactation
Pregnancy Category C. Abetolol should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, caution should be exercised when Abetolol is administered to a nursing woman. Consult your doctor before use if you are pregnant or breastfeeding.
