Overview of the medicine
Act 500 mg Tablet contains Paracetamol, which is used to relieve mild to moderate pain and reduce fever. It works by blocking the production of certain chemical messengers that cause pain and fever in the body.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg (1-2 tablets) every 4-6 hours as needed. Do not exceed 4000 mg (8 tablets) in 24 hours.
Elderly
Similar to adult dosage, but caution should be exercised in patients with impaired renal or hepatic function.
Renal_impairment
Dose interval should be increased in patients with severe renal impairment (e.g., 500 mg every 8-12 hours for CrCl <10 mL/min).
How to Take
Take Act 500 mg Tablet orally with a glass of water, with or without food. Do not crush, chew, or break the tablet.
Mechanism of Action
Paracetamol is thought to exert its analgesic and antipyretic effects primarily by inhibiting prostaglandin synthesis in the central nervous system, though its exact mechanism is not fully understood. It has minimal anti-inflammatory effect.

Onset
30-60 minutes for analgesic and antipyretic effects.
Excretion
Mainly renal, as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1-4 hours, may be prolonged in liver disease or overdose.
Absorption
Rapidly absorbed from the gastrointestinal tract, peak plasma concentrations occur within 30-60 minutes.
Metabolism
Primarily hepatic, conjugated with glucuronic acid and sulfuric acid; a small amount is metabolized via cytochrome P450 to a toxic intermediate (NAPQI) which is detoxified by glutathione.
Side Effects
Contraindications
- •Hypersensitivity to paracetamol or any component of the formulation.
- •Severe hepatic impairment or active liver disease.
Drug Interactions
Alcohol
Increases the risk of hepatotoxicity, especially with chronic excessive intake.
Warfarin
May increase the anticoagulant effect, leading to an increased risk of bleeding.
Cholestyramine
May reduce the absorption of paracetamol.
Metoclopramide, Domperidone
May increase the absorption rate of paracetamol.
Certain antiepileptic drugs (e.g., Carbamazepine, Phenytoin)
May increase the risk of paracetamol hepatotoxicity.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, abdominal pain, sweating, and loss of appetite, which may appear 24-48 hours after ingestion. Severe overdose can lead to hepatic necrosis, renal tubular necrosis, and coma. Treatment involves gastric lavage, activated charcoal if within an hour of ingestion, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation when taken at recommended doses. However, it is always advisable to consult a doctor before using any medication during these periods.
