Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system by inhibiting prostaglandin synthesis, which contributes to its analgesic and antipyretic effects.
Pharmacokinetics
Onset
Analgesic and antipyretic effects typically begin within 30-60 minutes.
Excretion
Mainly excreted in urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma elimination half-life is approximately 1-4 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver mainly by conjugation with glucuronic acid and sulfate. A small amount is metabolized via cytochrome P450 to a reactive intermediate (N-acetyl-p-benzoquinone imine or NAPQI), which is normally detoxified by conjugation with glutathione.
