Description
Overview of the medicine
Afixime 100 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, ear, nose, throat, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
Typically 400 mg daily as a single dose or in two divided doses for adults if suspension is used. Dosage duration depends on infection type.
Elderly
No special dosage adjustment needed unless renal function is severely impaired.
Renal_impairment
Creatinine clearance 21-60 mL/min: 300 mg daily. Creatinine clearance <20 mL/min: 200 mg daily.
How to Take
Administer orally, with or without food. Shake the suspension well before each use. Use a measuring spoon or oral syringe for accurate dosing.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the final transpeptidation step of peptidoglycan synthesis. This leads to the disruption of bacterial cell wall formation and eventual bacterial cell lysis.
Pharmacokinetics
Onset
Peak plasma concentrations usually occur 2-6 hours after administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and also via biliary excretion.
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% absorbed orally.

Not extensively metabolized; some metabolism occurs in the liver.

Contraindications
- Hypersensitivity to Cefixime or other cephalosporins
- History of serious hypersensitivity reactions to penicillin

Carbamazepine
May increase carbamazepine plasma concentrations. Monitor carbamazepine levels.
Warfarin and other anticoagulants
May increase prothrombin time/INR, increasing risk of bleeding. Close monitoring is advised.

Store dry powder below 30°C, protected from light and moisture. Store reconstituted suspension in a refrigerator (2-8°C).
