Overview of the medicine
Afixime 200 mg capsule contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, ear, and skin.
Uses & Indications
Dosage
Adults
Adults and children over 10 years or weighing more than 50 kg: 200-400 mg orally once daily or in two divided doses (e.g., 200 mg every 12 hours) depending on the severity of the infection. For uncomplicated gonorrhea, a single dose of 400 mg.
Elderly
No dosage adjustment necessary in elderly patients unless there is severe renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min: 300 mg daily. For creatinine clearance < 20 mL/min: 200 mg daily. Hemodialysis and CAPD patients: 200 mg daily.
How to Take
Afixime capsules should be taken orally with or without food. It is recommended to take at the same time each day to maintain consistent levels.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to bacterial lysis and death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the transpeptidation step of peptidoglycan synthesis.

Primarily excreted in urine (about 50% unchanged) and bile.

Absorption
Approximately 40-50% orally absorbed. Food slightly delays absorption but does not significantly affect the extent.

Limited hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or other cephalosporin antibiotics
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics
Drug Interactions
Warfarin
May increase prothrombin time (PT) and International Normalized Ratio (INR), increasing the risk of bleeding. Close monitoring of PT/INR is recommended.
Nifedipine
May increase Cefixime absorption and peak plasma concentration.
Carbamazepine
Cefixime may increase carbamazepine levels. Monitoring of carbamazepine levels is advisable.
Oral Anticoagulants (e.g., Rivaroxaban)
Increased anticoagulant effect, monitor INR.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment is primarily symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Excreted in breast milk in small amounts; caution advised during lactation.
