Description
Overview of the medicine
Amdocal 10 mg Tablet contains Amlodipine, a calcium channel blocker used to treat high blood pressure (hypertension) and certain types of chest pain (angina). It works by relaxing blood vessels so blood can flow more easily, lowering blood pressure and improving blood supply to the heart.
Uses & Indications
Dosage
Adults
Initial dose is usually 5 mg once daily; may be increased to 10 mg once daily if needed.
Elderly
Lower starting dose (e.g., 2.5 mg once daily) may be considered, with careful titration.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Amlodipine is a dihydropyridine calcium channel blocker that inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This causes peripheral arterial vasodilation and reduction in peripheral vascular resistance, leading to a decrease in blood pressure. It also dilates coronary arteries and arterioles, improving myocardial oxygen supply.
Pharmacokinetics
Onset
Gradual (6-12 hours for peak effect)
Excretion
Primarily excreted in urine (about 60% as inactive metabolites, 10% as unchanged drug).
Half life
30-50 hours
Absorption
Well absorbed orally, bioavailability 60-64%. Peak plasma concentrations within 6-12 hours.
Metabolism
Extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to amlodipine or other dihydropyridines
- •Severe hypotension
- •Cardiogenic shock
- •Unstable angina (unless in patients receiving other anti-anginal therapy)
- •Aortic stenosis (severe)
Drug Interactions
Sildenafil/Tadalafil
May cause additive hypotensive effects.
Statins (e.g., simvastatin)
Amlodipine may increase simvastatin exposure, limiting simvastatin dose to 20 mg daily.
CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
May increase amlodipine exposure, monitor for hypotension and edema.
CYP3A4 inducers (e.g., rifampicin, St. John's wort)
May decrease amlodipine exposure, monitor for reduced efficacy.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves supportive care, including gastric lavage, activated charcoal, IV fluids, and vasopressors if needed. Calcium gluconate may be administered intravenously.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Small amounts excreted in breast milk; caution advised during lactation. Consult doctor.
