Description
Overview of the medicine
Amoxicillin is a penicillin-type antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
250-500 mg (10-20 ml suspension) every 8 hours, or as directed by the physician.
Elderly
Adult dosage unless renal impairment is present, then dose adjustment is required.
Children
Children 20-40 mg/kg/day in 3 divided doses. For Amotid 125 mg/5 ml suspension, dosage is determined by age, weight, and severity of infection, as prescribed by a physician (e.g., 5 ml (125 mg) three times daily for younger children).
Renal_impairment
Dose adjustment is necessary for patients with severe renal impairment. Consult physician for specific guidelines.
How to Take
Shake the bottle well before each use. Administer orally with a measuring spoon or cup. Can be taken with or without food.
Mechanism of Action
Amoxicillin works by inhibiting the synthesis of the bacterial cell wall, leading to bacterial lysis and death. It is bactericidal.
Pharmacokinetics
Onset
Within 30 minutes to 1 hour.
Excretion
Primarily excreted unchanged in the urine by active tubular secretion and glomerular filtration. Renal excretion accounts for 60-70% of the dose.
Half life
Approximately 1-1.5 hours in individuals with normal renal function.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 1-2 hours after oral administration.
Metabolism
Approximately 10-25% of the dose is metabolized to inactive metabolites, primarily penicilloic acid.
Side Effects
Contraindications
- Known hypersensitivity to penicillin or other beta-lactam antibiotics.
- History of severe allergic reactions (e.g., anaphylaxis) to any beta-lactam agent.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. Close monitoring of INR is advised.
Probenecid
Increases and prolongs the plasma concentration of amoxicillin by reducing renal tubular secretion.
Allopurinol
Increased risk of skin rash when co-administered with allopurinol.
Oral contraceptives
May reduce the efficacy of oral contraceptives. Additional contraceptive methods may be needed.
Storage
Store the dry powder below 30°C, protected from light and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after the recommended period.
Overdose
Symptoms of overdose may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances in fluid and electrolyte balance. Management involves symptomatic and supportive treatment, including gastric lavage or induced emesis if recent ingestion. Hemodialysis can remove amoxicillin from circulation.
Pregnancy & Lactation
Amoxicillin is generally considered safe during pregnancy (Category B). It is excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but caution is advised. Consult a doctor.
