Overview of the medicine
Amoxicillin is a penicillin-class antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250 mg to 500 mg every 8 hours, or 500 mg to 875 mg every 12 hours, depending on the infection.
Elderly
Dosage adjustments may be needed based on renal function.
Renal_impairment
Dosage reduction required for severe renal impairment (e.g., CrCl <30 mL/min).
How to Take
Orally with or without food. Take at evenly spaced intervals.
Mechanism of Action
Amoxicillin works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located on the bacterial cell membrane, which are involved in the final stages of peptidoglycan synthesis, a crucial component of the bacterial cell wall. This disruption leads to osmotic instability and bacterial cell lysis.

Excretion
Primarily excreted unchanged in the urine via renal tubular secretion and glomerular filtration.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, stable in the presence of gastric acid. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Partially metabolized; about 10-25% of the dose is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin or other penicillins.
- History of severe hypersensitivity reaction to any other beta-lactam antibacterial agent.
Drug Interactions
Warfarin
May prolong prothrombin time (INR).
Probenecid
Decreases renal tubular secretion of amoxicillin, increasing amoxicillin blood levels.
Allopurinol
Increased incidence of skin rash, particularly in hyperuricemic patients.
Methotrexate
Amoxicillin may reduce renal clearance of methotrexate, potentially increasing methotrexate toxicity.
Oral Contraceptives
May reduce the efficacy of oral contraceptives.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms: Nausea, vomiting, diarrhea, crystalluria (in cases of severely impaired renal function or large doses). Management: Symptomatic and supportive treatment. Gastric lavage or activated charcoal may be considered if ingestion is recent. Hemodialysis can remove amoxicillin from the circulation.
Pregnancy & Lactation
Pregnancy: Category B. Generally considered safe, but use only if clearly needed. Lactation: Excreted in breast milk in small amounts; generally considered safe, but monitor infant for potential adverse effects like diarrhea, candidiasis, or allergic sensitization.
