Description
Overview of the medicine
Apimox 500 mg Capsule contains Amoxicillin, a broad-spectrum penicillin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
General: 250 mg to 500 mg every 8 hours, or 750 mg to 1 gram twice daily, depending on the severity and type of infection. Max: 6g/day.
Elderly
No specific dosage adjustment is required unless there is significant renal impairment.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For CrCl <30 mL/min, reduce dose or increase dosing interval. Consult physician.
How to Take
Apimox capsules can be taken with or without food. Swallow the capsule whole with a glass of water. Do not chew or crush.
Mechanism of Action
Amoxicillin exerts its bactericidal action by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which are essential for cell wall synthesis. This interference leads to osmotic instability and subsequent cell lysis.
Pharmacokinetics
Onset
Peak plasma concentrations are usually reached within 1-2 hours after oral administration.
Excretion
Primarily excreted by the kidneys; 50-70% of the dose is excreted unchanged in the urine within 6 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, approximately 70-90% bioavailable. Food does not significantly affect absorption.
Metabolism
Approximately 10-25% of the dose is metabolized to an inactive penicilloic acid in the liver.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin, other penicillins, or any component of the formulation.
- History of severe hypersensitivity reaction (e.g., anaphylaxis) to any beta-lactam antibiotic.
Drug Interactions
Probenecid
Increases and prolongs amoxicillin blood levels by reducing renal tubular secretion.
Methotrexate
Increases methotrexate toxicity due to reduced renal clearance.
Oral Contraceptives
May reduce efficacy of oral contraceptives (rarely significant, but advise additional contraception).
Anticoagulants (e.g., Warfarin)
May prolong prothrombin time; monitor INR closely.
Storage
Store at controlled room temperature (20°C to 25°C), away from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and rarely, convulsions. Treatment is supportive; hemodialysis may be used to remove the drug from circulation in cases of severe renal impairment.
Pregnancy & Lactation
Pregnancy Category B. Amoxicillin is generally considered safe for use during pregnancy. Small amounts are excreted in breast milk, but it is generally considered compatible with breastfeeding. Consult a doctor.

Lab Monitoring
- Renal function (especially in patients with pre-existing impairment or prolonged therapy)
- Liver function (in patients with pre-existing hepatic impairment)
- Complete blood count (during prolonged therapy)
Doctor Notes
- Always inquire about penicillin allergy history before prescribing.
- Consider renal function when dosing, especially in elderly patients.
- Educate patients on the importance of completing the full course of therapy.
Patient Guidelines
- Take the full course of antibiotics as prescribed, even if you feel better.
- Do not share your medication with others.
- Inform your doctor if you have a history of allergies, especially to penicillins or cephalosporins.
- Report any severe or persistent side effects to your healthcare provider.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
