Description
Overview of the medicine
Aripiprazole is an atypical antipsychotic medication used to treat certain mental/mood disorders such as schizophrenia, bipolar I disorder, and as an adjunctive treatment for major depressive disorder. It works by helping to restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial 10 or 15 mg once daily; target dose 15 mg/day. Bipolar I Disorder (Monotherapy): Initial 15 mg once daily. Bipolar I Disorder (Adjunctive): Initial 10 or 15 mg once daily. MDD (Adjunctive): Initial 2-5 mg once daily, target 5-15 mg/day.
Elderly
No specific dose adjustment is generally recommended based on age alone. However, consider lower initial doses and slower titration, especially if other risk factors are present.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take Aripiprazole 10 mg tablet orally once daily with or without food. Swallow the tablet whole with water. Do not crush, chew, or split the tablet unless specifically instructed by your doctor.
Mechanism of Action
Aripiprazole is a partial agonist at D2 and 5-HT1A receptors, and an antagonist at 5-HT2A receptors. This unique combination of actions is thought to contribute to its efficacy in treating psychotic symptoms and mood disorders.
Pharmacokinetics
Onset
While some symptomatic improvement may be observed within a few days, full therapeutic effects for chronic conditions may take several weeks.
Excretion
Approximately 55% of the dose is eliminated in the feces and approximately 25% in the urine, primarily as metabolites.
Half life
The mean elimination half-life is approximately 75 hours for aripiprazole and 94 hours for its active metabolite, dehydroaripiprazole.
Absorption
Well absorbed orally, peak plasma concentrations typically occur within 3-5 hours. Absolute oral bioavailability is approximately 87%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP3A4 enzymes. Dehydroaripiprazole, the active metabolite, is also formed.
Side Effects
Contraindications
- •Known hypersensitivity to aripiprazole or any of its excipients. Reactions have ranged from rash to anaphylaxis.
Drug Interactions
Antihypertensives
May potentiate orthostatic hypotension due to aripiprazole's alpha-adrenergic blocking activity.
Other CNS depressants (e.g., alcohol, benzodiazepines)
May potentiate the sedative effects of aripiprazole; advise caution.
Strong CYP3A4 inducers (e.g., carbamazepine, rifampin)
Double aripiprazole dose.
Strong CYP2D6 and CYP3A4 inhibitors (e.g., ketoconazole, quinidine, fluoxetine, paroxetine)
Reduce aripiprazole dose by half.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose typically include somnolence, vomiting, tremor, and tachycardia. Management involves supportive care, including maintaining an adequate airway, oxygenation, and ventilation. Cardiovascular monitoring and symptomatic treatment are recommended. Activated charcoal may be useful.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to antipsychotic drugs during the third trimester of pregnancy are at risk for extrapyramidal and/or withdrawal symptoms. Aripiprazole is excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
