Description
Overview of the medicine
Aristo-D3 40000 IU Capsule is a high-dose Cholecalciferol (Vitamin D3) supplement used to treat and prevent severe vitamin D deficiency and associated conditions.
Uses & Indications
Dosage
Adults
For severe vitamin D deficiency: 40,000 IU once weekly for 6-8 weeks, followed by maintenance therapy as advised by a physician.
Elderly
Similar to adult dosage, but caution should be exercised and serum calcium levels monitored regularly.
Renal_impairment
Use with caution and monitor serum calcium and phosphate levels closely. Dosage adjustment may be necessary based on severity and physician's advice.
How to Take
Take the capsule orally with water, preferably with a main meal to enhance absorption. Do not chew or crush the capsule.
Mechanism of Action
Cholecalciferol is converted in the liver to calcifediol (25-hydroxyvitamin D) and then in the kidneys to calcitriol (1,25-dihydroxyvitamin D), which is the active form of vitamin D. Calcitriol acts to regulate calcium and phosphate metabolism, promoting absorption from the gut, reabsorption in the kidneys, and mineralization of bone.
Pharmacokinetics
Onset
Therapeutic effects may take several days to weeks to become apparent, as it involves conversion to active forms and accumulation.
Excretion
Primarily excreted in bile and feces, with a small amount excreted in urine.
Half life
The half-life of 25-hydroxyvitamin D (calcifediol) is approximately 15-20 days. The half-life of the active form, calcitriol, is much shorter (4-6 hours).
Absorption
Well absorbed from the small intestine, primarily in the presence of bile salts and dietary fat. Peak plasma concentrations are reached within 4-5 hours.
Metabolism
Initially hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol), then further hydroxylated in the kidneys to 1,25-dihydroxyvitamin D (calcitriol).
Side Effects
Contraindications
- Hypersensitivity to Cholecalciferol or any components of the formulation.
- Hypercalcemia (high levels of calcium in the blood).
- Hypervitaminosis D (excessive levels of vitamin D in the body).
- Nephrolithiasis (kidney stones).
Drug Interactions
Corticosteroids
May reduce the effect of vitamin D.
Thiazide diuretics
May increase the risk of hypercalcemia.
Phenytoin, Barbiturates
May increase the metabolism of vitamin D, reducing its effectiveness.
Cholestyramine, Colestipol, Orlistat, Mineral oil
May reduce the absorption of vitamin D from the gut.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of vitamin D overdose (hypervitaminosis D) are primarily due to hypercalcemia, including nausea, vomiting, loss of appetite, constipation, weakness, fatigue, polyuria, polydipsia, and in severe cases, renal impairment, cardiac arrhythmias, and calcification of soft tissues. Management involves immediate discontinuation of vitamin D, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
During pregnancy and lactation, vitamin D supplementation should be undertaken with caution and only under strict medical supervision. Excessive doses can be harmful to the fetus or infant. It is secreted into breast milk.
