Description
Overview of the medicine
Aripiprazole is an atypical antipsychotic medication used to treat mental health conditions such as schizophrenia, bipolar I disorder, and as an adjunctive treatment for major depressive disorder. It helps to restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
For Schizophrenia: Initial 10-15 mg once daily, target dose 15 mg/day. For Bipolar Mania: Initial 15 mg once daily. For Major Depressive Disorder (adjunctive): Initial 2-5 mg once daily, target 5-15 mg/day. (Specific dosage for Arotril 5 mg tablet depends on the condition and doctor's assessment).
Elderly
Lower starting doses and careful titration are recommended, especially in elderly patients with dementia-related psychosis due to increased mortality risk.
Renal_impairment
No dosage adjustment is generally required for patients with renal impairment.
How to Take
Take Arotril 05 mg tablet orally once daily, with or without food, as directed by your doctor.
Mechanism of Action
Aripiprazole is believed to exert its effects through a combination of partial agonism at dopamine D2 and serotonin 5-HT1A receptors, and antagonism at serotonin 5-HT2A receptors.
Pharmacokinetics
Onset
Antipsychotic effects may take several weeks to fully develop; acute agitation relief may be seen sooner.
Excretion
Excreted primarily through feces (approximately 55%) and urine (approximately 25%).
Half life
Approximately 75 hours for aripiprazole and 94 hours for its active metabolite, dehydro-aripiprazole.
Absorption
Well absorbed after oral administration, with peak plasma concentrations reached within 3-5 hours.
Metabolism
Primarily metabolized in the liver by CYP2D6 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Known hypersensitivity to aripiprazole or any component of the formulation.
Drug Interactions
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine)
May increase aripiprazole plasma concentrations, requiring a dose reduction.
CYP3A4 inducers (e.g., carbamazepine, rifampicin)
May decrease aripiprazole plasma concentrations, requiring a dose increase.
CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin)
May increase aripiprazole plasma concentrations, requiring a dose reduction.
Storage
Store below 30°C, in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include somnolence, vomiting, tachycardia, tremor, and headache. Management is primarily supportive; activated charcoal may be considered. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Aripiprazole is excreted in human breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
