Description
Overview of the medicine
Cetirizine Hydrochloride is a non-sedating antihistamine used to relieve symptoms of allergy such as sneezing, runny nose, itching, and watery eyes. It is effective for both seasonal and perennial allergic rhinitis, as well as chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
10 mL (10 mg) once daily.
Elderly
No dose adjustment generally required, but consider reduced dose (e.g., 5 mL) if renal impairment is present.
Renal_impairment
For creatinine clearance 31-50 mL/min: 5 mL (5 mg) once daily. For creatinine clearance 11-30 mL/min: 5 mL (5 mg) every other day. For creatinine clearance <10 mL/min: Contraindicated.
How to Take
For oral use. Can be taken with or without food.
Mechanism of Action
Cetirizine selectively inhibits peripheral H1-receptors, thereby preventing the actions of histamine, which is primarily responsible for allergic symptoms. It does not readily cross the blood-brain barrier, leading to less sedation.
Pharmacokinetics
Onset
Within 20-60 minutes.
Excretion
Primarily excreted unchanged in the urine (approximately 60%) within 24 hours.
Half life
Approximately 8-11 hours in adults.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1 hour.
Metabolism
Limited hepatic metabolism; approximately 10% of the dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivatives, or to any component of the formulation.
- Patients with severe renal impairment (creatinine clearance less than 10 mL/min).
Drug Interactions
Alcohol
Concurrent use with alcohol or other central nervous system (CNS) depressants may cause additional reductions in alertness and impairment of CNS performance.
Theophylline
A minor decrease in cetirizine clearance has been observed with concomitant administration of theophylline (400 mg once daily).
Storage
Store in a cool, dry place below 25°C. Protect from light and moisture. Keep away from freezing.
Overdose
Symptoms of overdose may include somnolence, agitation, and tachycardia. Treatment should be symptomatic and supportive, including gastric lavage or induced emesis if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Avoid use during lactation as cetirizine is excreted in breast milk.
