Description
Overview of the medicine
Asiclin 150 mg Capsule contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typical Azithromycin adult dosage for most infections is 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days. For Asiclin 150 mg, the specific dosage regimen should be strictly as prescribed by a physician based on the infection type and severity.
Elderly
No dosage adjustment is generally required for elderly patients with normal renal and hepatic function. However, caution is advised due to potential increased risk of adverse reactions.
Renal_impairment
No dosage adjustment is required for mild to moderate renal impairment (creatinine clearance >10 mL/min). Use with caution in severe renal impairment as data is limited.
How to Take
Swallow the Asiclin capsule whole with a glass of water. It can be taken with or without food. To maximize absorption, it is often recommended to take macrolides 1 hour before or 2 hours after a meal.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby blocking translocation of peptides. This action is primarily bacteriostatic but can be bactericidal at higher concentrations.
Pharmacokinetics
Onset
Clinical effects may be observed within 2-3 days of initiation, depending on the infection.
Excretion
Primarily excreted unchanged in the bile, with only a small percentage (approx. 6%) excreted in urine.
Half life
Terminal elimination half-life is approximately 68 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations are reached 2-3 hours after administration.
Metabolism
Partially metabolized in the liver to inactive metabolites; CYP450 metabolism is minimal.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased digoxin levels; monitor digoxin levels and signs of toxicity.
Warfarin
Increased anticoagulant effect, monitor INR (International Normalized Ratio) closely.
Nelfinavir
Increased azithromycin serum concentrations; adjust azithromycin dosage if co-administered.
Antacids (containing aluminum or magnesium)
Decreased azithromycin absorption; administer azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store below 30°C in a dry place, protected from direct sunlight and moisture. Keep out of reach and sight of children.
Overdose
Symptoms of azithromycin overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and general supportive care are recommended as needed.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Azithromycin is excreted in human breast milk; use with caution during lactation, balancing the benefits of breastfeeding with potential risks.

YesClinical Trials
Azithromycin has been extensively studied in numerous clinical trials for its efficacy and safety across a wide range of bacterial infections. These trials have supported its use in respiratory, skin, and sexually transmitted infections. Ongoing research continues to evaluate its role in emerging infectious diseases.
Lab Monitoring
- Liver function tests (especially in patients with pre-existing hepatic impairment)
- INR (if co-administered with warfarin)
- ECG (in patients at risk of QT prolongation, e.g., with cardiac conditions or electrolyte imbalances)
- Renal function tests (in patients with severe renal impairment)
Doctor Notes
- Emphasize the importance of completing the full course of treatment to patients to minimize antibiotic resistance.
- Be cautious when prescribing Asiclin to patients with known QT prolongation, electrolyte disturbances, or those concurrently taking other QT-prolonging drugs.
- Advise patients to seek immediate medical attention if they develop symptoms of a severe allergic reaction or severe persistent diarrhea.
Patient Guidelines
- Always complete the full course of Asiclin, even if you start to feel better, to prevent bacterial resistance and ensure complete eradication of the infection.
- Do not share this medication with anyone else, even if they have similar symptoms.
- Report any severe or unusual side effects, especially persistent diarrhea, to your doctor immediately.
- Store the medicine as directed.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Asiclin may cause dizziness or visual disturbances in some individuals. If you experience these effects, avoid driving or operating heavy machinery until you are certain that your abilities are not impaired.
Lifestyle Advice
- Maintain good hand hygiene to prevent the spread of infection to others.
- Stay well-hydrated by drinking plenty of fluids.
- Avoid alcohol consumption during treatment, as it may exacerbate some side effects.
- Avoid prolonged sun exposure or use protective clothing/sunscreen as macrolides may cause photosensitivity.
