Description
Overview of the medicine
Asiclin 300 mg Capsule contains Clindamycin, a lincosamide antibiotic used to treat a wide variety of bacterial infections, including those of the respiratory tract, skin and soft tissue, bones and joints, and gynecological infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
150-450 mg every 6-8 hours, depending on the severity of the infection. For severe infections, 300-450 mg every 6 hours.
Elderly
No specific dosage adjustment required based solely on age; adjust based on renal/hepatic function if impaired.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe renal impairment (CrCl < 30 mL/min), dosage interval may need to be extended or dose reduced.
How to Take
Oral administration. Take capsules with a full glass of water, with or without food, to prevent esophageal irritation. Do not lie down for at least 30 minutes after taking the capsule.
Mechanism of Action
Clindamycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing peptide bond formation and subsequent bacterial growth and replication.
Pharmacokinetics
Onset
Therapeutic effects usually observed within a few hours.
Excretion
Excreted through urine (10-20% as active drug) and feces (about 4% as active drug), with the remainder as metabolites.
Half life
Approximately 2-3 hours in adults, may be slightly longer in neonates and patients with severe renal or hepatic impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract (approximately 90% bioavailability), with peak plasma concentrations occurring within 45-60 minutes after oral administration.
Metabolism
Primarily metabolized in the liver to active and inactive metabolites (e.g., N-demethyl clindamycin and clindamycin sulfoxide) by cytochrome P450 enzymes (CYP3A4 and CYP3A5).

Contraindications
- Hypersensitivity to Clindamycin or Lincomycin
- History of Clindamycin-associated colitis
- History of inflammatory bowel disease
Drug Interactions
Erythromycin
Antagonistic effects may occur, reducing clindamycin's efficacy.
Kaolin-pectin
May decrease the absorption of clindamycin if taken concurrently.
Neuromuscular blocking agents
May enhance the action of neuromuscular blocking agents, leading to increased or prolonged muscle paralysis.
Oral anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect, requiring close monitoring of INR.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, supportive and symptomatic treatment is recommended. Hemodialysis and peritoneal dialysis are not effective in removing clindamycin from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. In humans, clindamycin crosses the placenta. It is generally considered safe for use during pregnancy if clearly needed. Clindamycin is excreted in breast milk; therefore, caution should be exercised when administering to a nursing mother. Monitor breastfed infants for diarrhea, candidiasis, or allergic reactions.
