Description
Overview of the medicine
Asiclin 300 mg Injection is an antibiotic medicine that fights bacteria. It is used to treat a wide variety of bacterial infections, including serious infections of the skin, soft tissues, bone, joints, abdomen, female reproductive organs, and respiratory tract. It can also be used to prevent infections before certain surgical procedures.
Uses & Indications
Dosage
Adults
Serious infections: 600-1200 mg/day in 2-4 equally divided doses. More severe infections: 1200-2700 mg/day in 2-4 equally divided doses. Max single IM dose: 600 mg. Max daily IM dose: 1200 mg. IV infusion rate should not exceed 30 mg/min.
Elderly
No specific dosage adjustment required based on age alone, but monitor renal and hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment, consider extending dosing interval. Not significantly removed by hemodialysis.
How to Take
Asiclin 300 mg Injection is administered intramuscularly (IM) or intravenously (IV). For IV administration, it must be diluted and infused slowly over 10-60 minutes depending on the dose. It should not be administered as a bolus injection.
Mechanism of Action
Clindamycin is a lincosamide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible bacteria. This binding prevents the elongation of peptide chains, thereby inhibiting protein synthesis and leading to a bacteriostatic effect.
Pharmacokinetics
Onset
Immediate after IV administration; 1-3 hours after IM administration.
Excretion
Mainly excreted in urine and feces, with some excretion via bile. Approximately 10% of the dose is excreted unchanged in urine.
Half life
Approximately 2-3 hours in adults with normal renal and hepatic function.
Absorption
Rapidly and completely absorbed after intramuscular (IM) administration; peak serum concentrations achieved within 1-3 hours. Intravenous (IV) administration results in immediate peak levels.
Metabolism
Primarily metabolized in the liver to active and inactive metabolites (e.g., clindamycin sulfoxide, N-demethylclindamycin).
Side Effects
Contraindications
- Hypersensitivity to clindamycin, lincomycin, or any component of the formulation.
- History of antibiotic-associated colitis (especially C. difficile associated diarrhea).
Drug Interactions
Erythromycin
Antagonism between clindamycin and erythromycin has been demonstrated in vitro; concomitant use is not recommended.
Neuromuscular blocking agents
May enhance the neuromuscular blocking action, leading to increased weakness or paralysis.
Warfarin and other Vitamin K antagonists
May enhance the anticoagulant effects, requiring increased INR monitoring.
Storage
Store at room temperature, below 25°C (77°F). Protect from freezing. Keep in the original container to protect from light. Do not refrigerate.
Overdose
No specific antidote exists. Treatment of overdose should be symptomatic and supportive. Hemodialysis and peritoneal dialysis are not effective in removing clindamycin from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in pregnant animals showed no harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Clindamycin is excreted in breast milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Liver and kidney function tests (periodically during prolonged therapy)
- Complete blood count (CBC) (periodically during prolonged therapy)
- Stool examination for C. difficile toxin if diarrhea occurs.
Doctor Notes
- Always assess patient history for hypersensitivity to lincosamides or previous antibiotic-associated colitis.
- Monitor for signs of C. difficile-associated diarrhea during and after treatment.
- Consider renal and hepatic function, especially in elderly or critically ill patients, though significant dose adjustment is often not required.
Patient Guidelines
- Complete the full course of antibiotic treatment as prescribed, even if symptoms improve.
- Report any severe or persistent diarrhea to your doctor immediately.
- Inform your healthcare provider about all medications you are taking, including over-the-counter drugs and herbal supplements.
- Do not share this medication with others.
