Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For certain conditions, 500 mg (12.5 ml of 200 mg/5 ml suspension) once daily for 3 days, or 500 mg (12.5 ml) on day 1, then 250 mg (6.25 ml) once daily for 4 days.
Elderly
Dosage adjustments generally not necessary for elderly patients.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Administer orally, with or without food. Can be mixed with water. Ensure full course is completed.
Mechanism of Action
Inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby interfering with peptidyl transfer and preventing protein translation.

Variable, depends on the type and severity of infection.
Excretion
Primarily excreted unchanged in the bile; a small amount is excreted via the kidneys.
Half life
Long terminal elimination half-life of 2-4 days.
Absorption
Rapidly absorbed. Bioavailability is approximately 37%. Peak plasma concentrations are reached 2-3 hours after administration.
Metabolism
Partially hepatic metabolism, primarily by demethylation. Excreted largely unchanged.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Digoxin
Increased digoxin levels; monitor digoxin levels.
Warfarin
Increased anticoagulant effect; monitor INR.
Nelfinavir
Increased azithromycin concentrations; no dose adjustment needed usually.
Antacids containing aluminum or magnesium
May decrease peak serum levels of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store dry powder below 30°C, protected from light and moisture. Reconstituted suspension should be stored at room temperature (below 30°C) and used within manufacturer-specified period; do not freeze.
Overdose
Symptoms may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Treatment is symptomatic and supportive. Gastric lavage and supportive measures as required.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Azithromycin is excreted in breast milk; caution should be exercised when administered to a nursing woman.
