Overview of the medicine
Asizith 500 mg tablet contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections of the respiratory tract, skin, ears, and sexually transmitted infections.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days OR 500 mg on day 1, then 250 mg once daily for 4 days. For Chlamydia: 1g single dose.
Elderly
No dosage adjustment needed unless severe renal or hepatic impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment (CrCl >10 mL/min). Use with caution in severe impairment.
How to Take
Asizith 500 mg tablets can be taken with or without food. Swallow the tablet whole with water. Do not chew or crush.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby interfering with peptidyl translocation. This leads to inhibition of bacterial growth and replication.

Onset
Peak plasma concentrations reached in 2-3 hours.
Excretion
Mainly excreted unchanged in bile; some renal excretion.
Half life
Terminal elimination half-life is 2-4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.
Metabolism
Primarily metabolized in the liver, but to a limited extent.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Warfarin
Increased anticoagulant effect; monitor INR.
Nelfinavir
Increased azithromycin serum concentrations.
QT prolonging drugs
Increased risk of QT prolongation and Torsades de Pointes.
Antacids (aluminum and magnesium containing)
Decreased peak serum levels of azithromycin; take azithromycin at least 1 hour before or 2 hours after antacids.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Treatment is symptomatic and supportive; gastric lavage and general supportive measures may be indicated.
