Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days OR 500 mg on day 1, then 250 mg once daily for 4 days. For uncomplicated Chlamydial infections: 1 gram single oral dose.
Elderly
No dosage adjustment generally required for elderly patients.
Renal_impairment
No dosage adjustment necessary for mild-to-moderate renal impairment. Caution advised in severe renal impairment (CrCl < 10 mL/min).
How to Take
Oral, with or without food. Taking with food may reduce gastrointestinal upset.
Mechanism of Action
Inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with the translation of mRNA.

Onset
Clinical improvement typically observed within 2-3 days of initiating treatment.
Excretion
Mainly excreted unchanged in the bile; a small amount is excreted in urine.
Half life
Long terminal elimination half-life of 2-4 days, allowing for once-daily dosing and shorter treatment courses.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Primarily metabolized in the liver by N-demethylation.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Digoxin
May increase digoxin levels. Monitor digoxin levels.
Warfarin
May enhance the anticoagulant effect. Monitor INR closely.
Nelfinavir
Increased azithromycin serum concentrations. Monitor for increased side effects.
Antacids (aluminum and magnesium-containing)
May decrease peak plasma concentrations of azithromycin. Administer azithromycin at least 2 hours before or 4 hours after antacids.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management is symptomatic and supportive, as needed.

Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk, use with caution in lactating mothers due to potential for adverse effects on breastfed infant.
