Description
Overview of the medicine
Atorvastatin is used to reduce high cholesterol and triglyceride levels, and to prevent cardiovascular events.
Uses & Indications
Dosage
Adults
Initial dose is 10-20 mg orally once daily. Dosage can be adjusted based on response up to a maximum of 80 mg once daily.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take orally once daily, with or without food, preferably at the same time each day. It can be taken in the morning or evening.
Mechanism of Action
Atorvastatin inhibits HMG-CoA reductase, an enzyme responsible for cholesterol synthesis in the liver, leading to reduced LDL-C levels.
Pharmacokinetics
Onset
Lipid-lowering effects observed within 2 weeks, maximal effect by 4 weeks.
Excretion
Excreted primarily via bile after hepatic metabolism.
Half life
Approximately 14 hours for the parent compound, with active metabolites contributing to a longer half-life of inhibitory activity (20-30 hours).
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily hepatic metabolism by cytochrome P450 3A4 (CYP3A4) to active and inactive ortho- and para-hydroxylated metabolites.
Side Effects
Contraindications
- Active liver disease or unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Hypersensitivity to atorvastatin or any component of the formulation
Drug Interactions
Digoxin
May increase digoxin concentrations; monitor digoxin levels.
Warfarin
May cause an increase in INR; monitor INR closely.
Cyclosporine, Gemfibrozil, Niacin (lipid-modifying doses)
Increased risk of myopathy/rhabdomyolysis and elevated atorvastatin levels.
Clarithromycin, Erythromycin, Itraconazole, Ketoconazole (CYP3A4 inhibitors)
Significantly increased atorvastatin plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific treatment for atorvastatin overdose. In case of overdose, symptomatic and supportive measures should be instituted. Monitor liver function tests and CK levels.
Pregnancy & Lactation
Atorvastatin is contraindicated during pregnancy and lactation due to potential harm to the fetus/infant. Women of childbearing potential should use adequate contraception.

- Creatine kinase (CK) levels should be measured if muscle pain, tenderness, or weakness occurs.
Doctor Notes
- Emphasize the importance of adherence to diet and lifestyle modifications.
- Monitor liver function tests periodically, and creatine kinase if muscle symptoms develop.
- Counsel patients on potential drug interactions, especially with strong CYP3A4 inhibitors and grapefruit juice.
- Discontinue immediately if severe muscle symptoms or rhabdomyolysis is suspected.
Patient Guidelines
- Take Atv exactly as prescribed by your doctor.
- Do not stop taking Atv without consulting your doctor.
