Description
Overview of the medicine
Atorvastatin is a lipid-lowering medication belonging to the statin class. It reduces the levels of 'bad' cholesterol (LDL) and triglycerides in the blood, while increasing 'good' cholesterol (HDL), thus reducing the risk of heart disease and stroke.
Uses & Indications
Dosage
Adults
Initial: 10-20 mg once daily. Maintenance: 10-80 mg once daily. Max: 80 mg/day.
Elderly
No dose adjustment required based on age, but caution advised.
Renal_impairment
No dose adjustment necessary for renal impairment.
How to Take
Administer orally once daily, with or without food, at any time of day, preferably at the same time each day.
Mechanism of Action
Atorvastatin selectively and competitively inhibits HMG-CoA reductase, an enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis, thereby reducing cholesterol levels.
Pharmacokinetics
Onset
Lipid-lowering effects typically observed within 2 weeks, maximal effect by 4 weeks.
Excretion
Primarily via bile (fecal), less than 2% excreted in urine.
Half life
Approximately 14 hours for atorvastatin and its active metabolites.
Absorption
Rapidly absorbed, peak plasma concentrations within 1-2 hours. Bioavailability approximately 14%.
Metabolism
Primarily hepatic by CYP3A4 to active ortho- and para-hydroxylated metabolites and beta-oxidation products.
Side Effects
Contraindications
- Active liver disease, including unexplained persistent elevations of serum transaminases
- Pregnancy
- Breastfeeding
- Hypersensitivity to atorvastatin or any component of the formulation
Drug Interactions
Colchicine
Increased risk of myopathy when co-administered with statins.
Cyclosporine, Gemfibrozil, Niacin, Macrolide Antibiotics (e.g., Erythromycin), Azole Antifungals (e.g., Itraconazole), HIV Protease Inhibitors (e.g., Ritonavir)
Increased risk of myopathy/rhabdomyolysis due to increased atorvastatin plasma concentrations.
Storage
Store at controlled room temperature (20-25°C), away from moisture and light. Keep out of reach of children.
Overdose
No specific antidote. In case of overdose, provide symptomatic and supportive measures as needed. Hemodialysis is unlikely to significantly enhance atorvastatin clearance due to extensive protein binding.
Pregnancy & Lactation
Contraindicated during pregnancy and breastfeeding due to potential fetal harm (category X) and excretion into breast milk.

Lab Monitoring
- Lipid panel (before initiation and periodically thereafter)
- Liver function tests (ALT/AST) (before initiation and periodically during treatment, especially when dose is escalated)
- Creatine Kinase (CK) (if muscle pain or weakness occurs)
Doctor Notes
- Monitor liver function tests at baseline and as clinically indicated.
- Assess muscle symptoms (pain, tenderness, weakness) carefully and measure CK levels if indicated.
- Counsel patients on lifestyle modifications.
- Consider drug interactions, especially with CYP3A4 inhibitors.
Patient Guidelines
- Take as directed by your doctor, usually once daily.
- Do not stop taking the medication without consulting your doctor.
- Inform your doctor immediately if you experience unexplained muscle pain, tenderness, or weakness, especially if accompanied by fever or dark urine.
- Maintain a low-cholesterol diet and regular exercise as advised by your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Atorvastatin generally does not affect the ability to drive or operate machinery. However, if you experience dizziness, avoid driving or operating heavy machinery.
Lifestyle Advice
- Adopt a heart-healthy diet low in saturated and trans fats.
- Engage in regular physical activity.
- Quit smoking and limit alcohol consumption.
