Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria. This antibiotic treats only bacterial infections. It will not work for viral infections (e.g., common cold, flu).
Uses & Indications
Dosage
Adults
Typical dosage: 500 mg once daily for 3 days OR 500 mg on day 1 followed by 250 mg once daily for 4 days. For Chlamydia trachomatis: 1 gram as a single dose.
Elderly
No dosage adjustment necessary for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment. Caution advised in severe renal impairment (GFR < 10 mL/min).
How to Take
Take orally with or without food. To minimize gastrointestinal upset, it can be taken with food. Do not chew or crush the tablet.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting bacterial protein synthesis. It is generally considered bacteriostatic, but may be bactericidal at high concentrations.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-3 hours.
Excretion
Primarily excreted unchanged in bile and feces; a small amount (about 6%) is excreted in urine.
Half life
Terminal elimination half-life is 2-4 days.
Absorption
Well absorbed orally; bioavailability is approximately 37%. Food may decrease Cmax but overall absorption is not significantly affected.
Metabolism
Partially metabolized in the liver to inactive metabolites; major metabolic pathways are not fully elucidated.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any excipients in the formulation.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase serum digoxin levels. Monitor digoxin levels if co-administered.
Warfarin
May enhance the anticoagulant effect of warfarin. Close monitoring of Prothrombin Time/INR is recommended.
Nelfinavir
May increase azithromycin serum concentrations. No dosage adjustment is recommended.
QTc-prolonging drugs
Avoid co-administration with other drugs known to prolong the QTc interval (e.g., antiarrhythmics, antipsychotics) due to potential risk of cardiac arrhythmias.
Antacids (containing aluminum or magnesium)
May reduce the peak plasma concentrations of azithromycin. Administer azithromycin at least 2 hours before or 4 hours after antacids.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are typically extensions of known side effects, such as severe nausea, vomiting, diarrhea, and abdominal discomfort. In case of overdose, gastric lavage and general supportive measures are indicated. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Azithromycin is excreted in human breast milk; use with caution in nursing mothers.
