Overview of the medicine
Azaltic (Azithromycin) is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin infections, and certain sexually transmitted infections.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days OR 500 mg on day 1, then 250 mg once daily for 4 days. For uncomplicated Chlamydia: 1g single dose.
Elderly
No dosage adjustment is necessary for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment (creatinine clearance >10-80 mL/min). Use with caution in severe renal impairment (creatinine clearance <10 mL/min).
How to Take
Azaltic tablets and oral suspension can be taken with or without food. However, taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Azithromycin works by inhibiting bacterial protein synthesis. It binds to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with mRNA-dependent protein synthesis without affecting nucleic acid synthesis.
Pharmacokinetics
Onset
Peak plasma concentrations are generally achieved within 2-3 hours.
Excretion
Primarily excreted unchanged in the bile (about 50%), with a small amount excreted in urine (about 6%).
Half life
The terminal elimination half-life is approximately 2 to 4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.
Metabolism
Partially metabolized in the liver, primarily by N-demethylation.
Side Effects
Contraindications
- •Known hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Digoxin
Increased serum digoxin levels, monitor digoxin levels.
Warfarin
Increased anticoagulant effect, monitor INR.
Nelfinavir
Increased azithromycin concentration, monitor for side effects.
Ergot derivatives (e.g., ergotamine)
Potential for ergotism, concurrent use should be avoided.
Antacids (containing aluminum or magnesium)
Decreased peak azithromycin levels; administer azithromycin at least 2 hours before or after antacids.
Storage
Store below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves supportive measures and symptomatic treatment.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing woman.
