Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat various bacterial infections such as respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
For most infections, 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days. For Chlamydia, a single 1g dose.
Elderly
No specific dosage adjustment is necessary for elderly patients.
Renal_impairment
No dosage adjustment is required for mild to moderate renal impairment (creatinine clearance >10 mL/min). Use with caution in severe renal impairment.
How to Take
Administer orally, with or without food. For better absorption of the capsule form, it is recommended to take it on an empty stomach.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby blocking translocation of peptides.
Pharmacokinetics
Onset
Onset of action typically within 2-3 hours.
Excretion
Excreted primarily unchanged in the bile, with a small amount (about 6%) excreted renally.
Half life
Terminal elimination half-life is 68 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.
Metabolism
Primarily metabolized in the liver, with significant biliary excretion.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase digoxin serum concentrations.
Antacids
Concomitant administration with antacids may reduce the peak serum concentration of azithromycin. Take azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
May increase anticoagulant effect.
Nelfinavir
May increase azithromycin serum concentrations.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management should be symptomatic and supportive as indicated by the patient's clinical condition.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Excreted in breast milk, use with caution during lactation.
