Overview of the medicine
Azilsartan Medoxomil is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension). It helps to relax blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
The recommended starting dose is 80 mg once daily. A starting dose of 40 mg once daily may be considered if required.
Elderly
No initial dose adjustment is required for elderly patients. However, caution is advised.
Renal_impairment
No dose adjustment is needed for patients with mild to moderate renal impairment. For severe renal impairment, lower doses (e.g., 40 mg) may be considered, and careful monitoring is recommended.
How to Take
Azilpres tablets should be taken orally once daily, with or without food.
Mechanism of Action
Azilsartan Medoxomil is a prodrug that is rapidly hydrolyzed to azilsartan, the active moiety. Azilsartan selectively blocks the binding of angiotensin II to the AT1 receptor in many tissues, including vascular smooth muscle and the adrenal gland. This inhibition reduces the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation and decreased blood pressure.
Pharmacokinetics
Onset
Significant blood pressure reduction is observed within 2 weeks, with maximum effects usually achieved within 4-8 weeks.
Excretion
Approximately 55% of azilsartan is excreted in feces and 42% in urine, mainly as unchanged drug or inactive metabolites.
Half life
The elimination half-life of azilsartan is approximately 11 hours.
Absorption
Azilsartan Medoxomil is rapidly hydrolyzed to azilsartan during absorption from the gastrointestinal tract. Peak plasma concentrations (Cmax) of azilsartan are reached within 1-3 hours.
Metabolism
Azilsartan is metabolized to two primary inactive metabolites primarily via O-dealkylation by CYP2C9 and CYP2C8, though metabolism is minor.
Side Effects
Contraindications
- •Hypersensitivity to azilsartan medoxomil or any component of the formulation.
- •Second and third trimesters of pregnancy.
- •Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m²).
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity have been reported. Monitor lithium levels.
Aliskiren
Concomitant use is contraindicated in patients with diabetes or renal impairment due to increased risk of hypotension, hyperkalemia, and renal impairment.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal function deterioration, especially in elderly, volume-depleted, or renally impaired patients. Monitor renal function.
Potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium
May lead to hyperkalemia. Monitor serum potassium levels.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited data on overdose. The most likely manifestations would be hypotension and tachycardia; bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive. If ingestion is recent, gastric lavage and/or activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy: Contraindicated in the second and third trimesters due to fetal toxicity. Discontinue as soon as pregnancy is detected. Lactation: It is not known whether azilsartan is excreted in human milk. Due to the potential for adverse effects on the nursing infant, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
