Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted infections.
Uses & Indications
Dosage
Adults
500 mg once daily for 3 days or 500 mg on day 1 followed by 250 mg once daily for 4 days.
Elderly
No specific dosage adjustment required, but monitor for QT prolongation.
Children
For otitis media and pneumonia: 10 mg/kg once daily for 3 days. For pharyngitis/tonsillitis: 12 mg/kg once daily for 5 days.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment (CrCl >10 mL/min). Use with caution in severe impairment.
How to Take
Oral. Shake the suspension well before each use. Can be taken with or without food.
Mechanism of Action
Azithromycin works by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting protein synthesis.
Pharmacokinetics
Onset
Clinical improvement usually seen within 2-3 days of initiation.
Excretion
Excreted primarily via the bile into the feces; a small amount is excreted in urine.
Half life
The plasma elimination half-life is 2-4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin or any macrolide antibiotic
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Digoxin
May increase digoxin levels. Monitor digoxin levels.
Warfarin
Potentiates anticoagulant effect. Monitor INR closely.
Nelfinavir
Increases azithromycin serum concentrations. No dose adjustment needed but monitor for side effects.
Antacids containing aluminum or magnesium
Reduce peak plasma levels of azithromycin. Take azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store the dry powder below 30°C, away from moisture and light. Store the reconstituted suspension at room temperature (below 30°C) and discard any unused portion after 5 days.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and supportive care as needed.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Excreted in breast milk; caution advised during lactation.
