Bactazim 400 mg Capsule contains Cefixime, a third-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
Adults and children over 10 years or weighing more than 50 kg: 400 mg once daily or 200 mg twice daily. Duration of treatment depends on the infection, typically 7-14 days.
Elderly
No dosage adjustment is generally required in elderly patients with normal renal function.
Renal_impairment
For creatinine clearance 21-60 mL/min, the dose should be reduced to 300 mg daily. For creatinine clearance < 20 mL/min, the dose should be reduced to 200 mg daily. Patients on chronic ambulatory peritoneal dialysis (CAPD) or hemodialysis: 200 mg daily.
How to Take
Bactazim 400 mg Capsule can be taken orally with or without food. It is recommended to take it at the same time each day to maintain consistent levels of medicine in the body.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the transpeptidation reaction essential for peptidoglycan synthesis. This leads to the lysis and death of bacterial cells.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
The primary route of elimination is renal excretion, with about 50% of the absorbed dose excreted unchanged in the urine within 24 hours. Non-renal clearance also plays a role.
Half life
The plasma half-life averages 3 to 4 hours, but may be prolonged in patients with impaired renal function.
Absorption
Approximately 40-50% of an oral dose of Cefixime is absorbed. Absorption is not significantly altered by the presence of food.
Metabolism
Cefixime is not extensively metabolized in the body. No active metabolites have been isolated from human serum or urine.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
Concurrent administration may increase Cefixime peak plasma levels and AUC.
Carbamazepine
Elevated carbamazepine levels have been reported when co-administered with Cefixime. Therapeutic drug monitoring may be necessary.
Oral Anticoagulants (e.g., Warfarin)
Increased prothrombin time (PT) and international normalized ratio (INR), leading to increased risk of bleeding. Close monitoring is recommended.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in human milk; use with caution in nursing mothers.
