Overview of the medicine
Benprox 250 mg Suspension is an oral antibiotic containing Cefixime, a third-generation cephalosporin. It is used to treat a wide range of bacterial infections in children, including respiratory tract infections, urinary tract infections, and ear infections.
Uses & Indications
Dosage
Adults
Not typically used in suspension form for adults. For children, dosage is based on body weight, usually 8 mg/kg/day once daily or 4 mg/kg every 12 hours. Maximum 400 mg/day.
Elderly
No specific dose adjustment is required for elderly patients unless there is severe renal impairment.
Renal_impairment
Dosage should be adjusted in patients with creatinine clearance < 60 mL/min. Consult physician for specific dose adjustments.
How to Take
Administer orally with or without food. Shake the bottle well before each use. Use the provided measuring spoon or cup to ensure accurate dosing. Complete the full course of treatment as prescribed by the physician.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby preventing the cross-linking of peptidoglycan chains which are essential for the strength and rigidity of the bacterial cell wall. This leads to bacterial cell lysis and death.

Onset
Peak plasma concentrations are achieved 2 to 6 hours after a single oral dose.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. Renal impairment significantly increases the half-life.
Half life
The plasma half-life is typically 3 to 4 hours.
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly alter the extent of absorption.
Metabolism
Cefixime is not significantly metabolized in the liver.

Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to penicillin (e.g., anaphylaxis).
Drug Interactions
Probenecid
May increase Cefixime plasma concentrations by inhibiting renal excretion.
Carbamazepine
Increased carbamazepine levels have been reported when co-administered with Cefixime.
Warfarin and other anticoagulants
May increase prothrombin time (PT) and International Normalized Ratio (INR), leading to an increased risk of bleeding. Close monitoring is advised.
Storage
Store the unreconstituted powder below 30°C, protected from light and moisture. Store the reconstituted suspension in a refrigerator (2°C-8°C) and do not freeze.

In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment is primarily symptomatic and supportive.
