Betasef 250 mg capsule contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg daily given as a single dose or in two divided doses of 200 mg every 12 hours. For 250 mg capsules, a common regimen might be 250 mg twice daily, or as directed by physician based on infection severity.
Elderly
No dosage adjustment is generally required for elderly patients, unless there is significant renal impairment.
Renal_impairment
For creatinine clearance < 60 mL/min, dosage adjustment is necessary. For creatinine clearance < 20 mL/min, a dose of 200 mg once daily is recommended.
How to Take
Take the capsule orally with or without food, as directed by your doctor. Swallow the capsule whole with a glass of water. Do not crush or chew.
Mechanism of Action
Cefixime is a bactericidal antibiotic. It inhibits bacterial cell wall synthesis by binding to one or more of the penicillin-binding proteins (PBPs) which in turn inhibits the final transpeptidation step of peptidoglycan synthesis in the bacterial cell wall, thereby inhibiting cell wall biosynthesis. This leads to lysis and death of the bacterial cell.

Onset
Within 1-2 hours of administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile (about 10%).
Half life
Average plasma half-life is 3 to 4 hours, sometimes up to 9 hours in renal impairment.
Absorption
Approximately 40-50% orally absorbed, with peak serum concentrations achieved within 2 to 6 hours.
Metabolism
Not extensively metabolized; about 50% of the absorbed dose is excreted unchanged.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics
- •History of serious hypersensitivity reactions to penicillin
Drug Interactions
Nifedipine
May increase cefixime peak plasma concentration and AUC.
Carbamazepine
May increase carbamazepine levels when co-administered.
Warfarin and other anticoagulants
May increase prothrombin time (PT) with or without bleeding, so INR monitoring is essential.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Management should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in breast milk in small amounts; caution should be exercised when administered to a nursing mother.
