Overview of the medicine
Betasef 500 mg capsule contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections including those of the ear, nose, throat, lower respiratory tract, urinary tract, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
Adults and children over 12 years: 400 mg once daily as a single dose or 200 mg every 12 hours. For Betasef 500 mg capsule, typically one 500 mg capsule once daily.
Elderly
No dosage adjustment is generally required for elderly patients unless they have severe renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min, 300 mg daily. For creatinine clearance < 20 mL/min, 200 mg daily. For patients on chronic ambulatory peritoneal dialysis (CAPD) or hemodialysis, 200 mg daily.
How to Take
Betasef 500 mg capsule should be taken orally, with or without food, as directed by a physician. It is important to complete the full course of treatment even if symptoms improve.
Mechanism of Action
Cefixime is a bactericidal antibiotic. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to lysis and death of the bacterial cell.
Pharmacokinetics
Onset
Peak plasma concentrations are usually reached within 2-8 hours after oral administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. Some is also excreted in the bile.
Half life
Plasma elimination half-life is typically 3-4 hours in healthy individuals, but can be prolonged in renal impairment.
Absorption
Approximately 40-50% absorbed orally. Absorption is not significantly altered by the presence of food.
Metabolism
Cefixime is not extensively metabolized in the body; about 10% may be metabolized.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or other cephalosporins
- •History of severe penicillin allergy
Drug Interactions
Warfarin
May increase prothrombin time and INR, leading to increased bleeding risk. Close monitoring of coagulation parameters is recommended.
Probenecid
Increases cefixime plasma concentrations and prolongs its half-life by decreasing renal tubular secretion.
Carbamazepine
Concomitant use may lead to increased plasma levels of carbamazepine. Therapeutic drug monitoring may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
