Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
For adults and children over 10 years or weighing more than 50 kg: Usually 200-400 mg once daily or 100-200 mg every 12 hours. For 125 mg oral solution, specific pediatric dosing applies, often 8 mg/kg/day in two divided doses or 4 mg/kg every 12 hours.
Elderly
Dosage adjustment not generally required unless renal function is severely impaired.
Renal_impairment
For creatinine clearance 21-60 mL/min, total daily dose should not exceed 75% of standard dose. For creatinine clearance <20 mL/min, total daily dose should not exceed 50% of standard dose.
How to Take
Shake the bottle well before each use. Administer orally with or without food. Use a measuring spoon or oral syringe for accurate dosing, especially for children.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, thereby inhibiting cell wall biosynthesis and leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Primarily renal excretion (approximately 50% unchanged drug in urine), with some biliary excretion.
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% orally administered Cefixime is absorbed from the gastrointestinal tract, whether taken with or without food. The extent of absorption is not significantly affected by food, but Tmax may be slightly prolonged.
Metabolism
Limited metabolism; primarily excreted as unchanged drug.
Side Effects
Contraindications
- •Hypersensitivity to Cefixime or other cephalosporins
- •History of anaphylactic reactions to penicillin (due to potential cross-reactivity)
Drug Interactions
Probenecid
May increase cefixime plasma levels by decreasing renal excretion.
Carbamazepine
Increased carbamazepine levels have been reported with concomitant cefixime administration.
Warfarin and anticoagulants
May increase prothrombin time and INR; careful monitoring is advised.
Storage
Store the dry powder below 30°C in a cool, dry place, away from light and moisture. After reconstitution, store in a refrigerator (2-8°C) and do not freeze.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
