Description
Overview of the medicine
Bilastine is a non-sedating, long-acting antihistamine used for the symptomatic treatment of allergic rhinoconjunctivitis and urticaria.
Uses & Indications
Dosage
Adults
One 20 mg tablet once daily.
Elderly
No dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is required for mild to moderate renal impairment. Use with caution in severe renal impairment (creatinine clearance <30 mL/min).
How to Take
Take the tablet orally with water, preferably on an empty stomach. This means one hour before or two hours after food or fruit juice (especially grapefruit juice).
Mechanism of Action
Bilastine acts as a selective peripheral H1-receptor antagonist. It inhibits histamine-mediated effects, providing relief from allergy symptoms without significant central nervous system effects.

Onset
Onset of action typically occurs within 30-60 minutes.
Excretion
Approximately 95% of the dose is excreted unchanged in urine and feces (28% in urine, 67% in feces).
Half life
Elimination half-life is approximately 14.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 61%. Peak plasma concentration is reached around 1 hour.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to bilastine or any of the excipients.
- •Children under 12 years of age (for 20 mg tablets).
Drug Interactions
Diltiazem
Increases plasma levels of bilastine.
Erythromycin
Increases plasma levels of bilastine.
Ketoconazole
Increases plasma levels of bilastine.
Grapefruit juice
Significantly reduces the oral bioavailability of bilastine.
P-glycoprotein inhibitors
Coadministration with P-glycoprotein inhibitors may increase bilastine exposure.
Storage
Store in a dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment is recommended. There is no specific antidote for bilastine.
Pregnancy & Lactation
Limited data are available on the use of bilastine in pregnant women. Animal studies do not indicate direct or indirect harmful effects. It should only be used during pregnancy if the potential benefit justifies the potential risk. It is unknown whether bilastine is excreted in human breast milk, so caution should be exercised when administered to a nursing mother.
